1095-C FORM
Employee Important Notice
The Affordable Care Act (ACA)requires certain employers to offer health coverage to eligible full-time employees. As a result, the Internal Revenue Service (IRS) created the 1095-C form. The 1095-C form is an annual statement generated by Johnson County reporting the health care coverage certain employees were offered and/or provided during the applicable calendar year. Johnson County is required to provide the 1095-C forms to the IRS each year to confirm who had or was offered coverage.
While the employer mandate of the ACA is still in effect, the individual mandate was repealed by Congress resulting in not federal penalty on individuals for not having health insurance. Therefore, the 1095-C is for information only and individuals do not need to send it to the IRS with their tax return. Hence, individuals do not need to wait until they receive the 1095-c to file their taxes.
Any Johnson County active or former employees, and retirees can obtain a copy of their 1095-C form upon request. Please contact:
